    Accessible Information Standard Statement

    We want everyone we care for to understand their care and to be understood. This statement explains how we meet the NHS Accessible Information Standard, so that people with communication needs get the right support.

    The six steps we follow

    The Accessible Information Standard asks us to follow six steps. Here is what each one means in plain words.

    • Ask: at our first meeting, we ask about your communication and information needs. We ask how you prefer to be spoken to and how you like information given to you.
    • Record: we write down what you tell us in your care plan, so every carer knows about it.
    • Flag: we make a note on your record so that anyone who opens it can see your needs straight away, without having to ask you again.
    • Share: we share the information with the people involved in your care, such as your GP or district nurse, so they know how to communicate with you too.
    • Meet: we meet your needs. This might mean giving you information in large print, easy read, Braille or audio, arranging an interpreter, or having a family member present to help you understand.
    • Review: we check back with you over time. If your needs change, or if something is not working, we update your care plan so the whole team knows.

    The support we can arrange

    This might include:

    • Information in large print, easy read, Braille or audio.
    • An interpreter, including a British Sign Language interpreter.
    • A family member or friend present to help you understand.
    • Extra time and patience from your carer.
    • Information given in a different way if something is not clear.

    If your needs change

    If your needs change over time, or if we are not communicating in the way that suits you best, please tell us. We will update your care plan and make sure the whole team knows.
